BACKGROUND

Edmonton Branch awards are disbursed following the guidelines outlined below, based on submissions from the teacher and/or the student. Please read the following information carefully as awards are determined according to the following criteria.

WHICH EXAM SESSIONS ARE INCLUDED?

Sessions from December of the previous calendar year to August of the current calendar year are eligible for consideration, i.e. December 2016 to August 2017.

HOW TO SUBMIT MARKS

Two pieces of information are required for each award submission:

            1) a copy of the first page of the student’s exam evaluation, either practical or theoretical, that includes the student’s name, address, exam taken, exam session, and percentage;

            2) a signed “Disclosure of Information” form (2017 Appendix A)  that allows the mark to be made public. This form is found at the end of this document and requires the signature of the candidate’s teacher. Students must be studying with an ARMTA teacher at the time of their exam.

BY SIGNING THIS “Disclosure of Information” form, YOU AGREE TO ALLOW ANY PHOTOGRAPHS OR VIDEO TAKEN DURING THE EVENT TO BE DISPLAYED ON THE ARMTA EDMONTON BRANCH WEBSITE.

These two forms MUST BE SUBMITTED TOGETHER and may be submitted by either the teacher OR the student/parent.

Inquiries may be directed to Rosemarie rosehorne@shaw.ca  OR 587 520 4006

Teachers and winners will be notified ONLY BY EMAIL by November 1, 2017

Submissions must be received by October 15, 2017 and must use the Disclosure of Information form 2017.

There is a section under the “Awards” tab on the website where students and teachers can check that their submission has been received by the Awards convenor.
